Providing accurate information about local support is one of the most important fundamental requirements across health and social care delivery. However, the current models tend to be inefficient, uncoordinated and create confusing results.

DCP have developed an interoperable model enabling the various stakeholders to work as a team in creating reliable information about local support. This model has emerged over the last 5 years from working with a number of council and health partners.
